And while most are in the awe of whether the promises would really be true or not, Bollywood actress Priyanka has filled the form for the high speed internet and the form has gone viral.

Although, the authenticity of the form is not yet known, but the form sure has Priyanka’s passport size photograph, her name, address, father’s name and her signature.

One recently heard that Priyanka has signed one big Hollywood and one Bollywood project, the details about both Priyanka wants to keep under the wraps for a while now. Her TV series Quantico has made quite a hustle across the globe; with this Priyanka has garnered the kind of fame that not even Aishwarya Rai could garner.
